HOW CAN I OBTAIN MY PATIENT RECORD OR BILL

All persons assessed, treated, or transported by the Vineland EMS Division have a Patient Care Report (PCR) that is stored securely. Patients who are treated and/or transported may also receive a bill of service.
In order to ensure compliance with federal law, we require all patients requesting records or bills to fill out the Vineland EMS Authorization To Disclose Individually Identifiable Protected Health Information (PHI) form.

PROTECTED HEALTH INFORMATION FORM

Along with the completed PHI form, the applicant must provide the following information as appropriate:
A copy of a valid Government Issued ID (front and back).
If a minor, parent/guardian must provide a birth certificate.
If the individual is deceased, please provide power of attorney paperwork, or paperwork from the Surrogate’s office.
Completed forms and attachments may be emailed to emsrecords@vinelandcity.org, or hand delivered to:
